Working within an integrated team covering the Integrated Team Network of the Bridgend borough to offer adviceand support to adults over the age of 18 who need support to maintain their independence and wellbeing and help to achieve their agreed outcomes, in addition to supporting carers in their own right.
A recognised social work qualification e.g. C.Q.S.W., C.S.S. Dip, S.W. Degree in Social Work and registration with the Social care wales or equivalent is essential.
2 years post qualifying experience.
Knowledge of legislation and regulatory and policy framework underpinning Services for adults.
Experience and commitment of working within an integrated team.
Experience of working in a strength based model and person centred perspective.
Experience in supporting individuals to identify outcomes within a strength based model in line with the ethos of the Social Services and Wellbeing Act Wales (2014).
Experience in co-ordinating the agreed plans and monitor and review the effectiveness of the plans in line with the outcomes identified.
Experience of working with other agencies and professionals, to achieve the best outcomes for adults and their families.
Driving license with access to vehicle.
